● ราคาดังกล่าวรวมค่าแพทย์และค่าบริการโรงพยาบาล
● ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม
● เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้
● ไม่สามารถคืนเงินได้ในกรณีที่มาใช้บริการแล้ว
● ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้ จนกว่าจะมาเข้ารับบริการ
● ราคาดังกล่าวขอสงวนสิทธิ์สำหรับชาวไทยและชาวต่างชาติที่อาศัยอยู่ในประเทศไทยเท่านั้น
● สำหรับคำสั่งซื้อก่อนวันที่ 1 มีนาคม 2568 สามารถใช้ได้ถึงวันที่ 31 มีนาคม 2568
และคำสั่งซื้อตั้งแต่วันที่ 1 มีนาคม 2568 เป็นต้นไป สามารถใช้ได้ถึงวันที่ 30 มิถุนายน 2568
