- ราคาดังกล่าวรวมค่าแพทย์และค่าบริการโรงพยาบาล
- โปรแกรมนี้ไม่สามารถเข้ารับบริการได้ที่ได้โรงพยาบาลสมิติเวช สุขุมวิท หรือศรีนครินทร์โดยตรง
- ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม
- เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้

- ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้ จนกว่าจะมาเข้ารับบริการ

- Price includes doctor’s fees and outpatient service fees.
- Exclusions: This program cannot be used directly at Samitivej Sukhumvit or Samitivej Srinakarin Hospital.
- Services received in addition to those specified in the program are subject to additional costs.
- Products and services constitute one health examination program; cancellation or refund of any individual part of the program is not permissible.
- Coupon is not refundable after redemption.
- Samitivej Hospital will issue a receipt after the customer has received the services at the hospital.
