- ราคาดังกล่าวรวมค่าแพทย์ และค่าบริการโรงพยาบาล
- ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม
- เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้
- ไม่สามารถคืนเงินได้ในกรณีที่มาใช้บริการแล้ว
- ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้จนกว่าจะมาเข้ารับบริการ
- ราคาดังกล่าวขอสงวนสิทธิ์สำหรับชาวไทยและชาวต่างชาติที่อาศัยอยู่ในประเทศไทยเท่านั้น

Coupon condition
- *** Price includes doctor's result analysis fees but does not include consultation fees (in cases an irregularity is detected) or hospital service fees
- Price includes doctor’s fees and outpatient service fees.
- Services received in addition to those specified in the program are subject to additional costs.
- Products and services constitute one health examination program; cancellation or refund of any individual part of the program is not permissible.
- Coupon is not refundable after redemption.
