• ราคาดังกล่าวรวมค่าแพทย์และค่าบริการโรงพยาบาล

• ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม

• เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้

• ไม่สามารถคืนเงินได้ในกรณีที่มาใช้บริการแล้ว

• ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้ จนกว่าจะมาเข้ารับบริการ

• ราคาดังกล่าวขอสงวนสิทธิ์สำหรับชาวไทยและชาวต่างชาติที่อาศัยอยู่ในประเทศไทยเท่านั้น

•  สำหรับคำสั่งซื้อก่อนวันที่ 1 มิถุนายน 2568 สามารถใช้ได้ถึงวันที่ 30 มิถุนายน 2568

•  สำหรับคำสั่งซื้อตั้งแต่วันที่ 1 มิถุนายน 2568 เป็นต้นไป สามารถใช้ได้ถึงวันที่ 31 ธันวาคม 2568
